In order to protect the health and safety of employees, staff, and students, all those who access Pasco County School locations must self-screen for symptoms and other risk factors of COVID-19 daily. If you answer YES to any of the questions, you should NOT be on campus. Access the Visitor Screener or the Student Screener online. Employees should use the Employee Self-Screener.

Student Registration

Please complete the Form for Pre-registration. After you have completed the form there will be two different options you can choose from:

  1. Forms will be sent to the email you provided on the Pre-registration form. You can complete the school forms and email them back to atessier@pasco.k12.fl.us or fax them to 813-794-1991 with the required documents listed below.

  1. You can sign up for an appointment time to come in and complete the school forms and provide your required documents as listed below. Click on the following link to sign up:

The required documents we will need you to provide to complete the enrollment process are:

  • Birth certificate/ Passport
  • Medical Physical Form (dated and signed by a health professional within one (1) year of enrollment (dated after 8/10/2020))
  • Current Immunization Form
  • 3 Evidence of Residence:

__ Owned Residence– deed, mortgage, or property tax assessment records, copy of current utility (electric/water) bill or initial order of service, auto registration, FL driver’s license, FL ID card or voter registration as evidence that parent(s) own and lives at the residence.

__ Leased Residence– Current lease or rental agreement or a notarized letter from the landlord, and a copy of a current utility (electric/water) bill or initial order for service, auto registration, FL driver’s license, FL ID card or voter registration as evidence that parent(s) lives at the residence.
